The easiest open-source survey platform to self-host, with genuinely nice in-app, website, and link surveys that reach product-feedback jobs Typeform and Google Forms cannot. The catch is open-core: the survey editor is fully free under AGPL, but SSO, team roles, multi-language, and removing the branding need a paid Enterprise license key even on your own server.

The free AGPL core covers survey building, but enterprise plumbing (SAML/OIDC SSO, role-based access, contact segmentation, multilingual surveys, de-branding) sits behind a commercial Enterprise license key that ships locked inside the Docker image.
Alive and shipping fast, on the 4.x line with weekly releases from a venture-backed GmbH out of the 2023 GitHub Accelerator, so the bus factor is a company. Small teams that want zero ops land on the free managed cloud instead.

What it can’t do
- Give you SSO, SAML, OIDC, team roles, or contact segmentation on the free build; those are Enterprise Edition features that stay locked without a paid license key, even when you host it yourself
- Remove the Formbricks branding or send white-label email follow-ups without the Enterprise license, so the free self-host carries the vendor's name
- Run multi-language surveys on the community edition; multilingual survey support is gated behind the same Enterprise key
- Match the depth of a heavy market-research tool like Qualtrics or LimeSurvey for complex question logic and advanced survey methodology
- Take drive-by community pull requests freely; the team currently facilitates outside code contributions only as an exception, so this is not a hack-on-it-yourself project
- Install and upgrade itself; self-hosting is real Docker, Postgres, backup, and patching work, and the free cloud tier caps monthly responses if you want to skip that

- Open-core gating is aggressive for self-hosters, SSO, team roles, multi-language, and de-branding all need a paid Enterprise license key baked into the image
- Free managed cloud caps monthly responses, so growing teams get pushed toward the $74/mo plan or higher
- Analytics and reporting are strong but can be fiddlier to configure than lighter form builders
- Not built for deep market-research methodology, so Qualtrics and LimeSurvey users will hit ceilings on advanced logic
- Community code contributions are only accepted as an exception, which limits the usual open-source fork-and-fix loop
- Self-hosting is genuine ops work, hosting, database, backups, upgrades, and security patching all fall on you
What the community says
We use Formbricks at our agency, as well as for some clients. I preferred it to HeyForm and OpnForm but they're all good options. Formbricks has native NPS support in its reporting which won us over. That and it has a great UI!
I've used both Formbricks and HeyForm. Formbricks has great analytics but was a bit harder to configure. While HeyForm was super easy to set up, it just had simpler analytics. It depends on what you want. If you have time and want surveys go with Formbricks. If you want simple things like RSVPs or polls go with HeyForm.
Worth taking a look at LimeSurvey and OhMyForm too. Although FormBricks is probably one of the best.
